Undergraduate students who are carrying out research that will be publicly disseminated, including honors and/or funded research; and graduate students engaged in research, must complete this form. A copy will then be forwarded to the relevant (Undergraduate or Graduate) Committee for approval. Depending on the answers to these questions, you may or may not be required to seek approval from institutional review boards, as specified below.

  • Who will participate in this research?

(It may be relevant to signal regional, or ethnic identity, age range, social class, professional affiliation etc. whatever designations will indicate the nature of your participants and, by implication their readiness to understand the consequences of their participation.)

 

  • What are your plans for assuring that the participants understand the nature of your research and the consequences of their participation?

(In some case this will mean providing an "informed consent" form , which you should attach here. In other case, you may need to explain why written consent in not appropriate to your methodology)


 

  • What steps are you taking to assure the privacy and dignity of the participants?

(If you will photographing/recording/taping people and/or their environment, please indicate how you will obtain their permission. Also indicate the measures by which you will assure the anonymity of participants who wish not to be identified in your reports. )

 

  • What plans do you have for sharing the results of your research with the participants?

(Among other things, you should indicate whether participants are literate in the language you use to write up the results of your study).
